Age  - 26 years

Complains

Pain in the region of abdomen, ovaries and the back during the first day of menstrual cycle; the patient has been married

for 4 years, primary infertility, and constipation. 

Objective data of the examination

Endo-vaginal examination on 28th day of menstrual cycle – ovaritis; right ovary inflammation, 45х30,5х27,5mm;V-19,7cm3,

with inflammatory type, not uniform  follicles 25,5х13 mm, homogeneous content and 13-14 and diffusely located small

cystic formations. Left ovary 59х45, 5х 43,5 mm V-61,5cm3, with hemorrhagic 2-chamber cyst with sizes 52x42 mm.

Cervix - 30,5х5х24 mm.  Along the full length of the channel are visualized the hemorrhagic regions, maximal size 1.5x1 mm.

Colpocervicoscopy study – hyperplasia of the channels mucous. Consultation of mammalogist – diffuse -cystic mastopathy.

No viruses. Bacterioscopic smear – leucocytes 26-28, trichomonas, fungus, gram-positive cooci.

Kymopertubation – poorly conducting tubes. 

 

Diagnosis

Primary infertility, has been married for four years; chronic inflammation; follicular-hemorrhagic cyst of the left ovary;

poorly conducting ducts; diffuse-cystic mastopathy.

Age - 22 years

Complains

The patient has been married for one year, infertility, constant constipation.

Objective data of the examination

Ultrasound examination on 12th day of menstrual cycle. In the right ovary is visualized non homogeneous cystic formation

size 13x10 mm and some small cystic formations in both ovaries.   ASA-221.4U/ml. Opt < 70 (antispermal antibodies).

Anti-β -CG(Ab-with respect to chorionic gonadotrophin) N ≤1,0 the patient has +2,0. CA -125: patient’s - 44.4;

Norm - Opt-0-20; max-35 U/ml. Computer kymopertubation – the tubes of the uterus are poorly conducting,

the uterine contraction rate and the contraction activity are reduced, the tone of tube-uterine and isthmic-ampullar

sphincter is increased.   

Diagnosis

Primary infertility; tube obstruction; anovulatory cycle.

Age - 33 years

Complains

The patient has been married for  10 years, primary infertility;  hirsutism,  alopecia. Menstruation from 12 years of age,

irregular, only as a result of progesterone administration.  Colitis with constipation; fatigue and weakness in

extremities, swelling of legs and hands at the end of day; hemorrhoids with frequent hemorrhages; troubled sleep, irritability.

Objective data of the examination

Computer kymopertubation – the tubes of the uterus are not conducting. The tone of tube-uterine and isthmic-ampullar

sphincter is increased. Endovaginal examination – the last menstruation 2 months ago. The uterine body with the cervix 7.1 cm;

uterus 5,5х4,2х6,0cm;  cervix 3,7 cm. Width of the uterus 5 mm. The right ovary - 2,0х1,4х1,8 cm., follicular apparatus is

not manifested. The left ovary – 2.2x1.6x2.8 cm, follicular apparatus is not manifested. Hormonal study – PROLACTIN-195

Diagnosis

Oligomenorrhea, anovulation, uterine tubes are not condutcing, hirsutism, colitis – constipation, hemorrhoids, primary infertility.

Duration of the treatment

4 months

The first month of treatment – 5 injections of progesterone, fertilogen.

Treatment results

The uterine tubes are completely conducting after 3 months of treatment. Restoration of the menstrual cycle with follicle maturation.

Complains

Headache at the beginning and at the end of the menstruation. Secondary infertility. 

Objective data of the examination

Ultrasound study – uterine body has the spherical shape, adenomyosis of mixed echogenicity, nonhomogeneous myometrium.

Inflammatory process in both ovaries with numerous small cystic inclusions. Computer kymopertubation – the uterine tubes

are not conducting, the tone of tube-uterine and isthmic-ampullar sphincter is increased. No viruses. Bacteriscopic smear

- trichomonas, fungus,  leucocytes  17-18, gram-positive cocci. 

Diagnosis

Secondary infertility, adenomyosis, ovaritis, tube obstruction.

26 day courses three times

Treatment results

After 52 days of treatment: Hysterosalpingography – the uterine cavity has the shape of elongated triangle, slightly less

than normal volume; with simple contours, deviated to the right. The right tube is coiled, up and both tubes are conducting.

In the pelvic cavity there is scarring. After more 26 days of treatment: by computer kympertubation the tubes are conducting.

Ultrasound examination – the uterine body is of normal echogenicity with simple contours, deviated to the right, homogeneous

myometrium. Follicle in the right ovary with diameter of 19 mm (12th day of menstruation). Bacteriologic smear – clear. 

Notice

Temporarily stopped the treatment and became pregnant next month.

Patient - B.A.

Age - 22 years

Complains

Pains during first day of menstruation. Diarrhea, flatulence, infertility.

Objective data of the examination

Hysterosalpingography – the uterine cavity has the shape of triangle, deviated to the back, with increased volume. The left

tube is not conducting at the beginning of isthmic portion. The right tube is not conducting in the interstitial portion.

Specific analysis of cervicitis smear – gonorrhoea, trichomonas, fungus, mycoplasma, ureaplasma, gardnerella, diplococci,

no leucocytes. Antispermal antibodies: ASA234.1 opt < 70U/ml Anti-β-CG(Ab-in respect with chorionic gonadotropin) +4 N≤1,0

Diagnosis

The tubes are not conducting. Infertility
